When you schedule your consultation with one of our vein care specialists, you will be speaking with one of the region’s most experienced specialists in treating venous diseases. In addition to explaining your treatment options, we will help you understand the root cause of your condition so that you can take the proactive steps needed to prevent varicose veins from progressing.

If You Are a New Patient
New patient forms can be downloaded prior to the appointment. If you are unable to complete forms ahead of time, please arrive at least 15 minutes early to allow time to fill out the necessary registration forms.
If You Need to Cancel an Appointment
We know life happens and that sometimes appointments need to be cancelled or rescheduled. We kindly ask that you call us at least 1 day prior to your appointment and let us know. We will be happy to reschedule your visit for you.
We strive to make every visit efficient and effective, but when an appointment is missed or cancelled with less than 24 hours’ notice, it disrupts the schedules of our providers, ultrasound technicians, and other patients waiting for care. Consequently, a $100 No-Show Fee will be charged for each provider scheduled, which must be paid before booking future visits. This fee is your responsibility, is not covered by insurance, and if unpaid, is subject to collections. Because consistent attendance ensures fair access to care for everyone, patients with three missed appointments may be asked to transfer their records to another physician. This policy and fee are subject to change without notice.
Cancellations of Surgical and Vascular Procedures: If you need to re-schedule a surgical or a vascular procedure (EVLT/MICRO) you must provide 48 hours (2 days) notice. Patients who do not cancel their appointment within this time frame and do not show up for the procedure will be charged a $300 cancellation fee. This fee must be paid before a new appointment is scheduled. This fee is your responsibility and is not covered by insurance, and if not paid is subject to collections. This fee is subject to change without notice.
We understand that your time is valuable. We try our best to keep on schedule. Please bear with us if we are running behind schedule. Emergencies occasionally occur. We do not rush appointments and allow the necessary time to answer each patient’s questions.
We request that cell phones not be utilized during appointments.

If you have a life-or limb-threatening emergency, call 911 immediately. For all other questions, please call the main office at 303-449-8346 (VEIN).
If it is a true after-hours emergency and your question cannot wait until regularly scheduled office hours, ask to have the physician on call paged.
Please note that Boulder Vein Center only refills prescriptions during office hours. Prescriptions will not be filled after office hours or on weekends. We ask you to plan accordingly, and if your prescriptions are running low, please call our office during business hours.
